青春健康调查

每两年我们的第六次, 8th-, 10th- and 12th-grade students participate in the Washington State 青春健康调查. Our schools will be conducting the survey during one class period the week of Oct. 10-21. The survey is sponsored by the Department of Health, the Family Policy Council and other state agencies. The survey is anonymous and students may chose to opt out. A copy of the survey questions is available for parents to preview in the school office. Brochures were mailed to all families with detailed information about the survey.

How does 云顶 use this information?
The district uses this information to plan programs designed to support our youth in schools and the community and to reduce risks. The district 营养 and Fitness advisory uses the feedback to plan and measure changes in behavior related to food choices and activity. Our Safe and Drug Free 学校 grants require us to survey students and use best practice programs. Many of our schools use the survey as a planning and measurement tool for their School Improvement Plan.

The survey asks questions that are related to students' increased risk of injury, health outcomes and alcohol and drug use. 调查结果被学校使用, communities and the state and local health departments to plan programs to support our youth and reduce their risks.

The survey is voluntary and anonymous.

Survey questions come from several well-established surveys that have taken place across the nation and in Washington. Parents or guardians can see a copy of survey questions in the school office.

Who will be asked to take the survey?
学生 in grades 6, 8, 10 and 12 across Washington state.

是的! 学生 can skip questions and stop taking the survey at any time. 学生 can also excuse themselves by telling their teacher that they do not want to take the survey.

学生 not taking the survey will be provided with an alternative activity, such as reading or working in the library.

There is no penalty for not taking part in the survey. Your student's grades will not be affected.

如何保护学生身份?
学生 will not write their names anywhere on the survey and there are no codes or other information to match a survey to a particular student.

No one from the school will look at the survey answers. 学生 put completed surveys into an envelope that is sealed before it leaves the classroom.

Survey reports of results will not identify students.
