  1. All immunization records turned in to schools or childcare centers after August 1, 2020, are required by state law to be 医学上验证. This means immunization records turned in to the school must be from a health care provider, or you must attach paperwork from a health care provider to your handwritten form that shows your child’s records are accurate. 此外,您还可以提交一份免疫状态证明 (CIS) printed from the Immunization Information System (IIS), which will not have a provider's signature.
  2. 学生 who are making progress toward completing their immunization requirements will be granted conditional status for entry into school or childcare.
  3. Tdap的剂量要求从6-12级到7-12级不等. All students who are entering 7th through 12th grades must have one booster dose of the Tdap vaccine.
